What Wound Care Management Involves

Wound care and management is a comprehensive clinical service covering the assessment, treatment, and ongoing monitoring of acute or chronic wounds. It’s delivered by registered nurses trained in wound management, evidence-based dressing techniques, infection control, and the clinical judgement that supports proper healing.
Without skilled assessment and consistent management, wounds can become infected, delay recovery, or require hospitalisation. Proper wound care prevents complications, supports faster healing, and keeps participants comfortable through their recovery.
Wound care can be accessed through your NDIS plan to support recovery from surgery, manage chronic wounds like pressure injuries or diabetic ulcers, and prevent complications. Our registered nurses document everything properly to support your plan and ongoing funding needs.
Our wound care is delivered in the home, where you’re most comfortable, using best practice clinical guidelines and the right products for each wound type and stage of healing. The clinical rigour matches a hospital, the experience is far more personal.

What Happens, And What You May Be Asked

Proper wound care follows a clear clinical process. Here’s what our registered nurses will typically work through with you to support healing, prevent complications, and keep you comfortable.
01

Initial Wound Assessment

A thorough clinical assessment of the wound, including size, depth, tissue type, surrounding skin, and any signs of infection or complication.
02

Personalised Treatment Plan

A written plan tailored to your wound type, stage of healing, and overall health, covering dressing choice, frequency, and clinical goals.
03

Dressing And Wound Care Delivery

Regular visits to clean, dress, and care for the wound using best practice techniques and evidence-based products suited to your healing stage.
04

Infection Prevention And Monitoring

Ongoing monitoring for signs of infection, complication, or delayed healing, with early intervention to prevent escalation or hospital readmission.
05

Pain And Comfort Management

Practical support and clinical advice to manage pain, reduce discomfort, and keep you as comfortable as possible throughout the healing process.
06

Education And Coordination

Clear guidance for you, your family, and your support team on wound care between visits, plus coordination with GPs and specialists when needed.

Every assessment is delivered with absolute discretion, in your time, and without rushing through what matters.

Why We're Built for Complex Care

Most providers can describe their model in a brochure. Ours shows up in the everyday work. Here’s what that actually looks like.
01

Clinically Led from Day One

Our directors are registered nurses. Every placement begins with our clinical team in the home, understanding the environment before anyone else steps in.
02

Built Around the Person

We design supports around how each participant actually lives, observing routines and risks first, then bringing in the right team once foundations fit.
03

Leadership You Can Reach

Our directors stay close to the work. When you need a decision, you’re talking to someone who already knows the participant well.
04

Care That Holds It Together

From hospital discharges to housing transitions and conversations with Public Guardians, Trustees and allied health, we keep placements steady.
